Carl E Fackrell SM3 – entered Eternal Patrol October 12, 1943 while assigned to – USS Dorado (SS-248). While in route to the Panama Canal Dorado suffered 77 killed, all hands lost. A Naval Court of Inquiry concluded it was highly likely Dorado was sunk by friendly fire. An air attack was made by a PBM Mariner aircraft from Patrol Squadron 210 of Guantanamo Bay which had an incorrect description of a restricted Bomb Area that was designated to protect against such a misidentification.
